Summer 2009 Health Insurance Waiver
Participation in a summer health insurance program is compulsory for all new international students and all domestic students admitted to their program for the summer and enrolled in 6 or more credit hours. SAIC's health insurance program may be waived by printing, completing, and submitting a health insurance waiver form per the link below.
Note: The summer waiver form is for the summer term only. Students wishing to waive insurance for the fall and spring semesters must complete a separate online waiver in PeopleSoft Self-Service. Questions regarding all waiver processes can be directed to Student Financial Services, Sullivan Center, 36 S. Wabash Ave., Suite 1218.
Summer 2009 Health Insurance Waiver Form
Domestic students - 2008-2009
Unless
domestic students submit an insurance waiver through Self-Service**,
they will automatically receive SAIC's health insurance and the
fee for health insurance coverage will be charged to the student's
account each term.
If a student has comparable coverage and
wishes to have the School's insurance waived for the fall semester or
for the full year, a waiver must be completed online through
Self-Service** by the first day of classes for the fall semester.
Spring semester only waivers must be completed online through
Self-Service** by the first day of classes for the spring
semester. Letters and phone calls will NOT be accepted as insurance
waivers.
Health Insurance Coverage is also available, upon
request through Self-Service** to all domestic degree-seeking students
enrolled in less than 12 hours. The fee for health insurance is then
added to the student's account each semester for which coverage is
requested. Please request health insurance coverage online through
Self-Service** by the first day of classes for the fall semester.
Spring only request must be made online through Self-Service** by the
first day of classes for the spring semester.
Reminder:
If a domestic student registers for 12 or more credit hours and then
drops to less than 12 hours before the end of the add/drop period, the
health insurance charge will automatically be removed from their
account and health insurance coverage will not be provided. The health
insurance charge will be added back to the account if the student
requests health insurance online through Self-Service.**

Exchange and international students
Exchange
and International students should return their completed waiver forms
through Self-Service.** by the first day of classes for the fall semester or full year. Spring semester only waivers
are due by the first day of classes for the spring semester. Letters and phone calls will
NOT be accepted as insurance waivers.
International students
who have graduated and are on Optional Practical Training and who wish
to have SAIC health insurance coverage should return their completed
request forms to the Office of International Affairs by the first day of classes for the fall semester. Spring semester only request
forms are due in the Office of International Affairs by the first day of classes for the spring semester.
